Abstract Candidiasis is one of the most common fungal infections of oral cavity in humans, causing great oral discomfort, pain and aversion to food. To develop more effective antifungal systems for the treatment of oral candidiasis, an oral mucoadhesive wafer containing sertaconazole solid dispersion (STZ-SD) was developed in this study. Dispersion of STZ in Soluplus® as a solubility enhancement excipient was done by melting, solvent evaporation and freeze drying method at various STZ to Soluplus® ratios. The optimized STZ-SD was then incorporated in the sodium carboxymethyl cellulose (SCMC) gel, xanthan gum gel, or their combination to prepare the lyophilized wafers. The swelling capacity, porosity, and mechanical, release and mucoadhesive properties of the wafers, together with their antifungal activity, were then evaluated. The melting method sample with the ratio of 8:1 showed the best results in terms of saturation solubility and dissolution rate. The STZ-SD-composite wafer exhibited higher hardness and mucoadhesion, as compared to those made of the SCMC polymer. The STZ-SD-wafer also exhibited a greater antifungal effect when compared to the STZ-wafer. The present study, thus, suggested that the STZ-SD-wafer could serve as a novel effective delivery system for oral candidiasis treatment.

Abstract Periodontitis is an oral disease associated with inflammation and pain with swollen and bleeding gums. In the present study, dental pastes containing NSAIDs, namely, diclofenac sodium and nimesulide (1 % w/w) were prepared to treat periodontitis. Dental pastes of diclofenac sodium and nimesulide (1 % w/w) were prepared with/without mucoadhesive hydrocolloid polymers such as sodium carboxy methyl cellulose (NaCMC), hydroxyl ethyl cellulose (HEC) and methyl cellulose (MC) by conventional trituration method. The pH, drug content, viscosity, tube spreadability and tube extrudability of these prepared dental pastes were measured. These dental pastes of diclofenac sodium and nimesulide (1 % w/w) were characterized by FTIR analyses for drug-excipient compatibility. The in vitro drug releases from these dental pastes in 6.4 pH phosphate buffer solution displayed sustained release over longer period and the drug release rate was found to be decreased when the concentration of mucoadhesive polymer was increased. These dental pastes displayed good adhesion to the oral mucosa revealing more retention time in mouth when tested for ex vivo mucoadhesion using bovine cheek pouch. The stability study results reveal that the DC3 and NC3 dental paste formulations were found stable enough over a longer period in different storage conditions. The present study revealed that the prepared mucoadhesive dental pastes of diclofenac sodium and nimesulide (1 % w/w) had good adhesion with the oral mucosa to maintain consistent release of drugs over prolonged time.

RATIONALE: Acute promyelocytic leukemia (APL) is an uncommon subtype of acute myeloid leukemia (AML). M3v phenotype is a less common presentation of APL and these patients usually present with leukocytosis and abnormal promyelocytes that are characterized by sparse granulation and are less likely to have faggot cells with multiple Auer rods. Distinguishing M3v phenotype from acute febrile illness can be challenging as the diagnosis relies on examination of peripheral smear. PATIENT CONCERNS: Fifty-seven-year-old female who presented after recent trip to Dominican Republic for high grade fever and gum bleeding. She was exposed to patients with Dengue fever during her stay. At presentation, patient had leukocytosis, thrombocytopenia, and urinalysis showing bacteria and white cell. She was started on treatment for urinary tract infection. Patient remained febrile and thrombocytopenia worsened. On day 2, flow cytometry of the peripheral smear showed 43% medium sized blasts. Fluorescence in situ hybridization was positive for promyelocytic leukemia/retinoic acid receptor alpha. DIAGNOSES: The patient was diagnosed with APL. INTERVENTIONS: Patient was started on treatment with all-trans retinoic acid and arsenic trioxide along with supportive care OUTCOMES:: Patient had a favorable clinical response and her symptoms subsided. LESSONS: Flow cytometry of the peripheral smear is key to diagnosis of suspected APL. One must maintain high suspicion for this life-threatening condition as early diagnosis saves lives.

The palatogingival groove is a developmental anomaly that typically starts near the cingulum of the maxillary incisors and extends along the roots at varying lengths and depths. Severe grooves that extend to the root apex often lead to complex combined periodontal-endodontic lesions. There are various therapeutic options available for these cases; however, the prognosis is unfavorable. Here, we report the successful surgical treatment of 3 cases of maxillary lateral incisors with severe palatogingival grooves using intentional replantation with a 2-segment restoration method. The teeth were gently extracted, resulting in minimal damage to the periodontal ligament. Under a dental operating microscope, 3 mm of the root end was resected. The palatogingival groove was removed, and root-end preparation was performed with a #700 fissure bur. The groove cavity was connected with root-end cavity to form a class II cavity. The cavity was then filled using a 2-segment restoration method (ie, dividing the cavity into 2 parts by the cementoenamel junction, the coronal portion was filled with a flowable composite while the radicular portion, including the root-end cavity, was filled with bioceramics). The tooth was then replanted into its alveolar bone and splinted with a flexible splint for 7 days. The sinus tract was closed at the 1-week postoperative visit. During subsequent recalls, the teeth showed almost complete periapical healing. In summary, intentional replantation with a 2-segment restoration method is a viable treatment modality for single-rooted teeth with a severe palatogingival groove that extends to the root apex.

PURPOSE: This clinical study aimed to evaluate the long-term efficacy of reconstruction of the gingival interdental papilla via injection of hyaluronic acid gel. MATERIALS AND METHODS: Eight female participants with Class I or II gingival papilla loss in anterior sites were included in this study. The gingival biotype was evaluated previously. Hyaluronic acid gel was injected into the base of the deficient papilla, which was repeated twice at 3 and 6 weeks after the initial injection. The height of the gingival papilla and the area of papilla loss were examined on clinical photographs before treatment and 3, 6, and 12 months after treatment. The data were tested for a normal distribution and analyzed by the Wilcoxon signed rank test. P < .05 was considered significant. RESULTS: The height of the gingival papilla increased 0.311, 0.45, and 0.4 mm from baseline at 3, 6, and 12 months, respectively, after treatment (P < .05), whereas the area of the black triangle was reduced by 0.31, 0.41, and 0.36 mm2 at the same time points (P < .05). In addition, patients with a thick gingival biotype showed a better effect of treatment on the increase in the height of the gingival papilla and decrease in the area of the black triangle. CONCLUSIONS: Our study verified a remarkable effect of hyaluronic acid gel injection in restoring the deficient gingival papilla of the natural teeth, especially in patients with a thick gingival biotype.

Pachyonychia congenita (PC) is a rare autosomal dominant condition caused by heterozygous mutation in one of five keratin genes. The purpose of this paper is to report a five-day-old infant with PC whose initial presentation revealed multiple malformed natal teeth and gingival lesions on the alveolar crest. Further investiga- tions led to genetic molecular testing of the child and his parents, which revealed a de novo and novel missense variant of KRT17 (c. 307C>T, p. Arg103Cys), resulting in a non-conservative amino-acid substitution and a diagnosis of PC. This case high- lights the need for multidisciplinary care and the relevance of molecular investigations for patients with multiple natal teeth. (J Dent Child 2019;86(1):61-3)

O objetivo do estudo foi analisar os efeitos da técnica de preenchimento com ácido hialurônico em gel em pacientes com triângulos negros, quanto ao preenchimento da papila interdental, durabilidade e recidiva dos efeitos do preenchimento; e à percepção do paciente quanto ao triângulo negro e à aplicação do ácido hialurônico. A amostra consistiu em 51 sítios de aplicação de 15 pacientes com triângulos negros na região anterior superior ou inferior. Foi realizado tratamento periodontal não cirúrgico, instruções de higiene bucal personalizadas, exame clínico e exame periodontal. Foram realizadas radiografias periapicais digitais dos dentes adjacentes ao defeito gengival, como medida controle; e aquisição de imagens fotográficas, realizadas perpendicularmente aos dentes de interesse, com auxílio de um posicionador personalizado para cada paciente, padronizando a distância da tomada fotográfica. As imagens fotográficas foram inseridas no programa ImageJ para avaliação da área do espaço negro. Aplicaram-se, para cada paciente, questionários, sobre a percepção do triângulo negro e sua vontade de corrigi-lo, e sobre a percepção do procedimento. Foram injetados até 0,1 ml de ácido hialurônico de cross-linking elevado 2-3 mm apical à extremidade mais oclusal da papila interdental em três tempos, com três semanas de intervalo entre eles. As respostas locais foram bastante variáveis entre indivíduos, locais de aplicação, e tamanho dos triângulos negros, havendo desde nenhuma modificação até a eliminação total do espaço. Todas as variáveis das profundidades de sondagem tiveram valores significativos na comparação de T0 a T4. A distância entre o ponto de contato e a crista óssea não mudou ao longo do tempo. A taxa de preenchimento da papila interdentária foi bastante variável, variando de nenhum preenchimento ao preenchimento completo da papila. Durante o período observado, os resultados obtidos foram mantidos. Os pacientes se declararam bastante incomodados com a presença do triângulo negro, e apesar da sensação dolorosa durante a aplicação se mostraram satisfeitos com o resultado e informaram que se submeteriam novamente ao procedimento. O AH injetável pode ser um tratamento promissor para melhorar a estética papilar. (AU)


The objective of this study was to analyze the effects of the hyaluronic acid gel filling technique in patients with black triangles regarding interdental papilla filling, durability and recurrence of filling effects; and the patient's perception about the black triangle and application of hyaluronic acid. The sample consisted of 51 sites of application from 15 patients with black triangles located in the upper or lower teeth in anterior region. Non-surgical periodontal treatment, personalized oral hygiene instructions, clinical examination and periodontal examination were performed. Digital periapical radiographs of the teeth adjacent to the gingival defect were performed as a control measure; and acquisition of photographic images, performed perpendicular to the teeth of interest, with the aid of a customized positioner for each patient, standardizing the distance of the photographic socket. The photographic images were inserted in the ImageJ program to evaluate the black space area. For each patient, questionnaires were applied on the perception of the black triangle and its willingness to correct it, and on the perception of the procedure. Up to 0.1 ml of cross-linking hyaluronic acid raised 2-3 mm apical to the most occlusal end of the interdental papilla in three times, with a three-week interval between them, were injected. All variables of probing depths had significant values in the comparison of T0 to T4. Distance between the point of contact and the bone crest did not change over time. The filling rate of the interdental papilla was quite variable, ranging from no filling to full filling of the papilla. During the observed period the obtained results were maintained. Patients were very uncomfortable with the presence of the black triangle, and despite the painful sensation during the application, they were satisfied with the results and would undergo the procedure again. Injectable hyaluronic acid gel may be a promising treatment for enhancing papillary esthetics. (AU)

Gingival Stillman's cleft is a mucogingival triangular- shaped defect which influence on health and appearance of gingiva. Due to a lack of clinical reports, five cases of gingival Stillman's cleft were recruited in the present study. The treatment methods were selected according to the etiological factors of the defect in each patient. Satisfactory treatment responses were achieved in all cases during a 6 to 20 month follow up after the treatments. In this study, we summarized the treatment strategies in various types of gingival Stillman's cleft, so as to provide the reference for clinicians.

O plano de tratamento para a exposição gengival excessiva depende da determinação precisa de importantes estruturas anatômicas através do exame clínico periodontal. A tomografia computadorizada é um exame que produz uma série de radiografias transversais que, posteriormente, são processadas por um computador, melhorando a precisão diagnóstica. Este artigo apresenta um relato de caso clínico sobre o planejamento terapêutico de exposição gengival excessiva, decorrente da erupção passiva alterada, utilizando o exame tomográfico. O exame tomográfico foi realizado com a paciente utilizando um retrator labial de plástico. A análise desse exame complementar demonstrou que a coroa clínica real era maior que a coroa aparente. Adicionalmente, revelou que a distância entre a junção cemento esmalte e a crista óssea alveolar era de 1 mm em média, indicando a ressecção óssea. O exame tomográfico com o uso de retrator labial permitiu maior qualidade e precisão na análise das estruturas anatômicas, auxiliando no diagnóstico, na determinação do prognóstico e no planejamento do tratamento da exposição gengival excessiva.


The treatment plan for excessive gingival display depends on the precise determination of important anatomical structures through periodontal clinical examination. The computed tomography is an examination that produces a series of transverse radiographies that are later processed by a computer, improving diagnostic accuracy. This article presents a clinical case report on the therapeutic planning of excessive exposure, due to altered passive eruption, using tomographic examination. The tomography exam was performed with the patient using a plastic labial retractor. The analysis of this complementary exam showed that the actual clinical crown was larger than the apparent crown. Additionally, it was found that the distance between cemento-enamel junction and the alveolar bone crest was 1mm on average, indicating bone resection. The tomographic exam with the use of a labial retractor allowed greater quality and precision in the analysis of the anatomical structures, aiding diagnosis, in the determination of the prognosis and in the planning of the treatment of excessive gingival display.

RESUMO

This case report presents two cases of rare congenital malformation, i.e syngnathia. First case is of 2-day infant with bilateral fusion of maxilla and mandible, leaving a small anterior portion. After consultation with other concerned specialties, early intervention was planned and fusion was released to facilitate feeding. The infant suffered from frequent respiratory tract infections and subsequently died at the age of ten months. The second case is of 8-month baby girl with unilateral congenital maxillomandibular bony fusion without any other anomaly. She underwent general anesthesia for thorough examination and release of soft tissue union. Second surgery was performed after few months for removal of bony fusion. Good mouth opening was seen on 1 month follow-up.

The frenum is a mucosal fold that attaches the lips or cheeks to the alveolar mucosa, gingiva, and underlying periosteum. Consequences of an abnormal frenal attachment include gingival recession, decreased vestibular depth, decreased range of lip movement, and involvement of interdental papilla, causing a diastema. Several methods to eliminate ectopic frenal attachments have been suggested, including frenectomy (elimination) and frenotomy (repositioning). This case report describes the use of a modified frenectomy technique in a 15-year-old girl with excess gingiva between the maxillary central incisors, which exhibited a 3-mm diastema. First, a semilunar primary incision was made in the palatal surface at a 5-mm distance from the tip of the papilla. Next, sulcular incisions were made around the tooth, and the papilla was transposed to the buccal via a papilla preservation flap. After complete elimination of frenal attachments in the bone, the flap was repositioned and sutured to the palatal surface. Afterward, the frenum was classically cut and sutured. Through this approach, the position of the frenum was changed apically without invading the papilla. At the 3-month follow-up, it was found that the modified technique (combination of papilla preservation flap and frenotomy) had minimized the surgical scar on the buccal surface, preserved the papilla, and yielded optimal esthetic results.

A estética diz respeito à percepção que cada indivíduo tem da beleza e é extremamente subjetiva, dessa forma compreender as expectativas de beleza do paciente é extremamente importante e muitas vezes difícil, devido ao fato de a percepção estética ser permeada por emoções e valores que diferem entre os indivíduos. A equipe interdisciplinar deve munir-se de todas as ferramentas possíveis para melhorar a visualização dos problemas estéticos, criar possíveis soluções, apresentar essas soluções para o paciente e guiar com precisão os procedimentos clínicos e laboratoriais para atingir resultados reabilitadores previsíveis e de sucesso. O objetivo deste relato de caso clínico é descrever um caso de reabilitação estética com laminados cerâmicos associados à cirurgia plástica periodontal.


Aesthetics refers to the individual's perception of beauty and it is extremely subjective, so understanding the patient's expectations of beauty is extremely important and sometimes difficult due to the fact that aesthetic perception is permeated by emotions and values that differ among individuals. The interdisciplinary team should have all possible tools to improve the visualization of aesthetic problems, create possible solutions, present these solutions to the patient and accurately guide clinical and laboratory procedures to achieve predictable and successful rehabilitative results. The objective of this clinical case report is to describe a case of aesthetic rehabilitation with ceramic laminates associated to periodontal plastic surgery.

Leiomyosarcoma (LMS) is a rare mesenchymal malignancy, of which 3-10% of cases occur in the head and neck region. We report a 22-year-old woman who was referred to the University Hospital of Siena, Italy, in 2016 with an ostensibly benign asymptomatic lump on the mandibular gingiva. The lesion grew rapidly, causing otalgia in the right ear. An excisional biopsy was performed and primary LMS was diagnosed histologically. Subsequently, the patient underwent radical re-excision of the perilesional mucosa, a partial bone resection and the extraction of four teeth. No recurrences or metastases were detectable at a 20-month follow-up. This report discusses the differential diagnosis of LMS with regards to other benign and malignant lesions and reviews the recent literature on primary and secondary oral LMS. Due to its innocuous clinical features-including its asymptomatic nature and presentation at a young age-this aggressive malignancy can go undetected; therefore, an early histopathological diagnosis is crucial.

O presente artigo teve como objetivo esclarecer as diferentes alternativas utilizadas para o reposicionamento labial e melhora da estética do sorriso, propondo uma nova classificação para o diagnóstico e tratamento do sorriso gengival. A técnica cirúrgica descrita no relato de caso associa o recontorno gengival e o reposicionamento labial com tratamento estético restaurador. A cirurgia realizada proporcionou a correção da erupção passiva alterada dos dentes anterossuperiores, combinada com o alongamento do feixe lateral do músculo levantador do lábio superior e da asa do nariz, e a contenção do mesmo com sutura mononylon, diminuindo o sorriso gengival. Notou-se, após a cirurgia, que a linha alta do sorriso foi corrigida sem comprometer a harmonia labial. A paciente manifestou alto grau de satisfação com o tratamento. O diagnóstico e a técnica cirúrgica proposta neste estudo podem ser uma opção terapêutica para o reposicionamento labial e harmonia do sorriso.


The present article aims to address the different alternatives used for lip repositioning and improved smile aesthetics, proposing a new classification for the diagnosis and treatment of the gummy smile. The surgical technique described this a case report associates gingival recontouring and lip repositioning with restorative aesthetic treatment. The surgery provided the correction of altered passive eruption of the anterior superior teeth combined with lengthening of the lateral bundle levator muscle of the upper lip and the ala of the nose, containing them with mononylon sutures, decreasing the gummy smile. It was noted that after surgery, the high smile line was corrected without compromising the labial harmony. The patient expressed high satisfaction with the treatment. The diagnosis and surgical technique proposed in the present study may be a therapeutic option for lip repositioning and smile harmony.

AIM: Surgical and orthodontic treatment of a teenage cleft patient. CASE REPORT: Authors describe the case of a 13 year old female cleft patient presented with class III malocclusion RESULT: The patient underwent comprehensive surgical secondary bone grafting and orthodontic treatment. Stable skeletal and occlusal class I relationship was achived and maintained in the post treatment observation period till the age of 16. DISCUSSION: Although several authors suggests primary gingivoperiosteoplasty, other advocates that such early intervention can cause later restrictions in maxillary growth. For alveolar reconstruction, maxillary growth and dental age were the main considerations in determining the timing of surgical intervention. CONCLUSION: This case showed that borderline cases of complex dentoalveolar and skeletal anomaly in cleft patients could be successfully treated with comprehensive secondary bone grafting and orthodontic treatment thus avoiding the need for orthognatic surgery. A palatogingival groove is an anatomic malformation that predisposes the involved tooth to a severe periodontal defect. When the condition is complicated by pulpal necrosis, affected teeth often present a dilemma in terms of diagnosis and treatment planning. In this report, we describe the case of a patient with a maxillary lateral incisor with a deep palatogingival groove extending to the root apex and severe periodontal destruction (local pocketing). Suggested treatment modalities included curettage of the affected tissues, elimination of the groove by grinding and/or sealing with a variety of filling materials, and surgical procedures. In this case, a combined treatment approach, involving both endodontic therapy and intentional replantation after restoration with a self-etching flowable composite, resulted in periodontal healing and significant healing of the periradicular radiolucency at 12 months. In short, intentional replantation offers a predictable procedure and should be considered a viable treatment modality for the management of palatogingival grooves, especially for single-rooted teeth.

A congenial syngnathia is very rare condition. It can be simple mucosal fusion (synechiae), or complete bony fusion (synostosis) between the maxilla or zygoma and the mandible. Fusion of the ascending ramus of mandible to maxilla and zygoma is less common than fusions of the alveolar ridges of the mandible to the maxilla. Bony syngnathia is either isolated or complex in form. There are 59 cases of congenital bony syngnathia reported in the literature: the first report was by Burket in 1936. There are 16 reported cases of zygomatico-maxillo-mandibular fusion. In the reported cases, women expressed the isolated form more commonly whereas men demonstrated a more complex pattern of disease. The authors present another patient of bony syngnathia involving bilateral fusion of the ascending ramus and body of the mandible with the maxillary complex in a young man. Early surgery was performed to release the bony and soft tissue fusion on the eighth day from the baby's birth. A second operation was performed for recurrence when the baby was 2.5 months old. A customized splint, an intense postoperative program of mouth exercises, and close follow-up aims to prevent further refusion.

A palatogingival groove is a developmental anomaly that predisposes the involved tooth to develop a severe periodontal lesion. These grooves often present a clinical challenge because diagnosis and treatment planning require an interdisciplinary approach. This case report describes the successful management of a right maxillary lateral incisor with a deep palatogingival groove in combination with an extensive periodontal pocket and pulp necrosis of the involved tooth. Collaborative management used a combination of endodontic treatment, periodontal therapy, odontoplasty, and a periodontal regenerative procedure using protein complex derived from enamel matrix (Emdogain; Straumann, Basel, Switzerland). Despite a predicted poor prognosis, the tooth lesion healed. This report also discusses the rationale behind the treatment modalities.
